Personal Release
By purchasing a ticket for a class I grant Artist Uncorked, its representatives, and employees the right to take photographs of me and my property. These images may be used in such purposes as publicity, illustration, advertising, and web content.
Cancellation Policy:
Due to the number of no shows, pre-registration is REQUIRED.
You may receive 100% refund if you cancelled before 24 hours of the scheduled class time.
Classes cancelled the day of will receive a credit towards another class.
No show = no refund or credit.
If I cancel the class all will receive 100% refund.
Liability Waiver:
By purchasing a ticket for a class I am intending to be legally bound, I hereby certify that if I am drinking:
1. I am 21 years of age or older.
2. I am submitting this release, waiver of liability, and assumption of risk declaration voluntarily and of my own free will.
3. The organizers, establishments, sponsors, and volunteers of this event are not responsible for any injury, loss or damage of any kind sustained by any person while participating in the Artist Uncorked classes or events, including injury, loss or damage which might be caused by the action, inaction, or negligence of others.
